While we make every effort to make your care affordable and help you understand the costs involved with working with us, it’s ultimately your responsibility to understand what your insurance covers and does not cover. If a service is performed at FLOURISH that your insurance does not cover, it will become your personal financial responsibility to pay for the services rendered.
It’s always a good idea to call your insurance ahead of coming in to verify your coverage. You can also use the provider search engine tool on your insurance website to verify that I am an In-network provider for you.
To assist you with calling your insurance, here are some tips for verifying your insurance benefits:
What to have ready when you call your insurance:
~The patient’s full name and date of birth.
~The primary insured’s name, address, date of birth, and social security number.
~The insurance ID number and GROUP number.
What to ask when you call your insurance:
~Is Jennifer Owen, Nurse Practitioner an in-network or out-of-network provider for you? (Please note: I am listed under Nurse Practitioner and NOT naturopathic medicine.)
~What are the effective dates for your coverage?
~Are the services you are seeking covered?
~Is any type of pre-authorization required for any services?
~Are there any limitations on the types or number of services you may receive for coverage?
~Is there any extra documentation required from us?
~What is your co-pay and/or out-of-pocket expense going to be?
~Are there any services that are not-covered?
~What is the preferred lab to use for your blood testing?
***Important! If you are coming to Jen for primary care, you may need to inform your insurance of this prior to your visit. Some insurances will not pay for primary care services if the provider is not on file with them. Please ask about any procedures you need to follow to ensure your visit will be covered.
